The Role
As a valued member of the Community Nursing Team, you’ll be managing a delegated caseload, providing complex, high-quality care to patients in their own homes and community settings. With guidance from experienced District Nurse Team Leaders, you’ll help shape and deliver care plans tailored to each patient’s unique needs.

Your Day-to-Day Will Include:
- Assessing, planning, and delivering person-centred care based on robust, evidence-based clinical practice
- Working independently and confidently in the community, with support available when needed
- Collaborating with patients, families, carers, and other professionals to deliver integrated care
- Playing a key role in enhancing standards and contributing to innovation in community health
- Keeping clear, accurate records while ensuring effective risk management and care delegation

What You’ll Need to Succeed
Essential:
- NMC Registered Nurse – Adult (Level 1)
- Strong clinical knowledge and ability to apply theory to practice
- A passion for community care and patient-centred outcomes
- Full UK driving licence or ability to travel efficiently between sites
Desirable:
- Experience in community nursing or acute services
- Teaching, assessing, or mentorship experience
- Understanding of clinical governance, health & safety, and risk management
- Welsh language skills – desirable, but not essential (language support available)
